Once the Folio CD-ROM has been set up on your workstation, start building your query by selecting either "Doctors" or "Facilities"

Searching by Doctor

Begin by selecting a category from the pick list. Some of the categories that you may select from are; Specialty, Town/Distance, Zip/Distance, State, Facility Type.

Once you have selected all the criteria for your query, click on the Run Query button to the top-right. This will show you the number of records returned as well as a result list with only the Name, Town, State and Zip fields showing.

Clicking on any of the physicians in the results list will display a detailed view of that physician.

Once you are satisfied with your results, you may then Export the list to your computer by clicking on the "Export" button. On the Export screen you can select the type of file you would like to export to, and select the fields that you are interested in.
