RESTRICTIONS: Physician and healthcare information provided by this site may not be downloaded, republished, reproduced, reprinted, retransmitted, distributed, rebroadcast, disseminated, circulated, duplicated or resold in whole or in part, without the expressed permission of Site Owner. Use of the databases to establish independent data files or compendiums of information is strictly prohibited and violators will be traced and prosecuted. The user agrees to access the information contained in the site manually and not programmatically by macro or other automated means.

SOURCE COPYRIGHTS: FolioMed receives its data from multiple sources, including but not limited to, federal, state, local and commercial sources, as well as directly from physicians and other healthcare providers. User acknowledges that all data is "Copyright" and "All Rights Reserved" by FolioMed. This site may be used only for individual use, and users may not sell, re-distribute or otherwise use the data for commercial purposes. Violators may be subject to criminal penalties. The Site Owner may use advanced tracing technology to prosecute violators.
